I hereby request inclusion in the MultiSport program and the issuance of a MultiSport card.
I acknowledge that my personal data will be processed for the purpose of administration, records, activation, deactivation and management of the MultiSport card, communication with the service provider Benefit Systems Slovakia s.r.o., payment records and the handling of possible complaints.
In connection with the provision of the MultiSport benefit, I acknowledge that:
- the controller of my personal data is the Slovak-Austrian Chamber of Commerce (SOHK),
- Matea s.r.o., Bytčická 89, 010 09 Žilina, Company ID: 53053966, acts as a processor of personal data and administers the MultiSport program on behalf of the controller,
- my personal data may be provided to Benefit Systems Slovakia s.r.o. to the extent necessary for the issuance and management of the MultiSport card,
- the following data in particular will be processed:
- first name and surname,
- email address (for a virtual card),
- data necessary for records and management of the MultiSport card,
- data on payments related to the use of the MultiSport card,
- for a children’s card, the child’s month and year of birth,
- personal data will be stored for the duration of the use of the MultiSport benefit and subsequently for the period required by the relevant legal regulations.
I also acknowledge that:
- the price of the MultiSport card and the administrative fee are paid to Matea s.r.o., which administers the program on behalf of the Slovak-Austrian Chamber of Commerce,
- failure to pay the price of the card or the administrative fee may lead to deactivation of the MultiSport card.
I confirm that I have been informed of my rights under Regulation (EU) 2016/679 of the European Parliament and of the Council (GDPR), in particular the right of access to personal data, rectification, erasure, restriction of processing, data portability and the right to lodge a complaint with the Office for Personal Data Protection of the Slovak Republic.
